How to enrol as a new Patient
Our Enrolments at Counties Medical Papakura and Takanini are open.
If you would like to enrol to see one of our GPs you can:

Call in to our Papakura or Takanini Clinics and one of our staff will be happy to assist you.
Phone 09 299 9384 and ask for our online link to be text or emailed to you.
Email us and we can send you the link to enrol online or email you a paper form to print and complete.
Thank you for your online enrolment form. To enable us to complete your enrolment please fill out a New Patient Medical Questionnaire. Either print or scan and return it to us at reception@countiesmedical.co.nz, or call into either clinic and fill out a paper copy of the questionnaire.
Patients under 16 years of age must have the child questionnaire completed.
Patients 16 years of age or over must complete their own form.
